How did birth control become health care, and then become an entitlement?

While the entire political world has been focusing obsessively for the last two weeks on the Catholic aspect of the issue of the contraceptive products and services provided under Obamacare, why is no one questioning the larger fact that Obamacare, which is supposedly about health care for Americans who can’t afford it, makes contraception free for everyone? Where did this idea come from, that every woman in America should have totally free access to birth control pills and other contraceptive services, without even a co-pay?

I have come upon an at least partial answer, and will post about it tomorrow.
